Creatine has an awkward relationship with the bathroom scale.

It can make body weight rise.

That fact is sometimes translated into two completely different claims:

“Creatine just makes you hold water.”

or:

“Every pound you gain on creatine is new muscle.”

Neither is a good description of what the evidence actually measures.

Early weight gain often includes water

Creatine increases creatine stored within skeletal muscle.

Changes in muscle creatine are accompanied by changes in water.

NIH’s Office of Dietary Supplements identifies weight gain related to water retention as one of the consistent effects seen with creatine supplementation.

That is especially relevant when somebody uses a loading protocol and changes muscle creatine stores relatively quickly.

So an early change on the scale should not automatically be interpreted as new muscle tissue.

Water inside muscle is still body mass

The phrase “just water” can also be misleading.

Body water is a real component of body composition.

And creatine-associated water retention does not mean the same thing as generalized pathological swelling.

The important point is simply that scale weight cannot identify which tissue or compartment changed.

For that, studies need body-composition measurements.

Resistance-training studies show more than an acute water effect

A 2024 systematic review and meta-analysis examined 12 resistance-training studies in adults younger than 50.

Compared with resistance training alone, creatine plus resistance training produced about 1.14 kg greater lean-body-mass gain on average.

The analysis also reported small reductions in body-fat percentage and fat mass.

That does not mean 1.14 kg can automatically be described as newly created contractile muscle.

Lean body mass includes water and other non-fat tissues.

But it also makes the opposite claim — that creatine-associated weight change is simply fat or meaningless water — difficult to defend.

A larger 2025 analysis points in the same general direction

A later systematic review pooled 61 controlled trials examining creatine supplementation alongside resistance training.

The analysis found an increase in fat-free mass and body mass, without significant overall increases in fat mass or body-fat percentage.

Again, terminology matters.

Fat-free mass and muscle mass are related concepts, not perfect synonyms.

A well-written article should preserve that distinction instead of turning every body-composition outcome into “muscle gained.”

Does creatine make you gain fat?

Creatine contains no meaningful calorie load capable of directly explaining fat gain.

The available training meta-analyses do not suggest that creatine supplementation systematically increases body fat.

Of course, a person’s body fat can increase while taking creatine if overall energy intake exceeds expenditure over time.

That would be an energy-balance issue occurring while using creatine, not evidence that creatine itself converts into body fat.

Why different people see different scale changes

Individual responses can vary because of:

  • starting muscle-creatine stores;
  • dietary creatine intake;
  • body size and muscle mass;
  • loading versus non-loading protocols;
  • training;
  • carbohydrate and fluid intake;
  • measurement timing.

Someone beginning creatine and resistance training simultaneously also creates a measurement problem: several things can change at once.

Loading makes the early change easier to notice

A conventional creatine-loading protocol can raise muscle creatine faster than a smaller daily dose.

That is useful when the objective is rapid saturation.

It also means the accompanying water-related change may happen quickly enough to be noticeable on the scale.

Taking a smaller daily amount without loading generally reaches the same conceptual destination more gradually.

The scale is the least specific measurement

If somebody gains 1.5 kg after starting creatine, the scale cannot tell us whether that consists of:

  • water;
  • muscle tissue;
  • glycogen-associated water;
  • fat;
  • gut contents;
  • some combination.

That is why single-person scale anecdotes are weak evidence about body composition.

A better interpretation uses time, training performance and appropriate body-composition measurements together.

The useful conclusion

Creatine can increase body weight.

Some early increase is commonly related to water retention.

Across resistance-training studies, creatine also tends to produce greater gains in lean or fat-free mass than training alone.

The evidence does not support treating creatine weight gain as synonymous with fat gain.

And it is equally inaccurate to call every new gram on the scale pure muscle.
